HELP! Interdisciplinary care for patients with cancer

Hi, I am trying to get info from nurses regarding working with:

PT

Dieticians

social workers

psych specialists

UA personnel

Oncology nurses

-on dealing with cancer patients. I know this is a very vague request- but if anyone is either a oncology nurse or works with oncology, if you can give me insight into how you deal with one or any of the above personnel in cancer patient care it would be appreciated.

Any info or experiences would be appreciated. I am doing a project and do not have time to get to the hospital this weekend to try to hunt anyone down that can help. Also it is the weekend, so may not find all of these people there anyway. If I had more than the weekend I could have probably got this done the traditional way :)...

How do you plan care for medicare patients vs. insured?

How does case managment benefit a patient?

Any info on discharge planning for the home health patient?
