help with failure to thrive work sheet

Hi All I am an educator for Hospice nurses and planning to re-educate our nurses on doing skin fold measures for failure to thrive, unfortuantely I can't seem to find Nomogram that will work. I have the worksheet for medicare but am missing the part on how to determine FTT on a patient that your unable to weigh. Could someone help? Thanks Kerri

At my agency, we only use FTT for a patient who can be weighed currently and has documented weights for 6 & 12 months prior. Otherwise, we would use the DX of Debility.

I would like to know the answer to this also. We just picked up a patient who is a double amputee. Does have heart issues but not staged as a class IV as of yet. I'm not sure what they picked her up under but we were discussing AFTT and decided that we wouldn't be able to get an accurate BMI r/t her amputation.

The LCD for FTT states that arm measurements or other may be documented on recerts but for initial cert you should have a BMI. We only use FTT if we have a BMI (and of course it has to be less than 22), otherwise we use debility unspecified. We were told by a surveyor that we could be denied payment if there isn't a documented BMI for a FTT pt- no matter how obvious it is that the pt is eligible for hospice. We don't use this diagnosis very often since many of our patients cannot be weighed.
