Help with diagnosis for care plan: Imbalanced nutrition

I'm working on a careplan for a type 1 DM patient. He was admitted for hyperglycemia. He hasn't checked his BS levels or took insulin in 2 years. He doesn't currently watch what he eats.

I'm wanting to use imbalanced nutrition as one of my diagnosis' but I'm having problems wording it. This is what I've come up with.

Imbalanced nutrition: more than body requirements related to patient not consuming proper diet, checking blood glucose leves, or taking insulin as manifested by elevated blood glucose levels.

What do you think? I'm feel okay as for the interventions I have, just wanted feedback on my wording. Any other advice is more than welcome also! Thanks.:)

Did you explore why he hasn't taken his insulin or checked his blood glucose in more than two years?

Imbalanced Nutrition: More Than Body Requirements is an appropriate nursing diagnosis for the fact the he doesn't watch what he eats, but not for the BG monitoring and not taking insulin. I would also consider Noncompliance (if he's been properly educated regarding his disease process and still chooses to not monitor his levels or take insulin) or Ineffective Therapeutic Regimen Management , as these would actually be at the root of the problem (i.e., he's hyperglycemic because he chooses to be noncomplaint with treatment; or he's hyperglycemic because he's not managing his prescribed treatment regimen properly).
