health promotion teaching plan for diabetes

I am having major trouble writing my health promotion teaching plan for diabetes type 2! My nursing dx is inneffective health maintenence r/t denial of need to change currrent habits.

My goal is Patient will describe positive health maintence behaviors and follow treatment regimin.

I have to come up with three outcomes (cognitive,affective, and psychomotor) with rationale and two interventions for each

I have googled evrything for ideas but I am stuck!

Any advice would be great! Please help!

I suggest starting by making a list of all the stuff a diabetic patient needs to know (cognitive), do (psychomotor), and value/feel (affective). It should be easy to find information about what diabetics need to learn -- just group them into the 3 categories. Pick the most important ... and you are halfway there.

I would also suggest searching the nursing literature (CINAHL ... school library, etc.) and not just the general public literature that you will get with google. A med/surg textbook would also have the information you need as would some of the better websites for people with diabetes.

Thank you for the reply! I did all the reseach and wrote a 4 page paper about the disease process I am just having trouble with the wording I guess! care plans are not my strong point! But im getting there...
