Isn't one needed? Does anyone happen to know off the top of their head how recent it needs to be when you apply (within 6 months, ect?) I was going to get one next week (I have to pay out of pocket- my work insurance isn't great at all) and I was wondering if I could kill 2 birds with 1 stone. Any insight? Thanks.

It has to be within one year of your CPNE date. My MD filled out the paper work based on my last physical which was october, my CPNE was July. By the way, the MD office did not charge to do the paper work:)
