Does anyone have a policy on "Hand-Off Communications" that they would like to share?:nurse:

I'd love to hear more, too. We just started a policy at the HH agency I work for but the written policy was confusing... to me and my supervisor! So far we're trialing it on some clients but not all so I haven't seen much as far as results or how other nurses are interpretting the policy. I'm sure at the end of the month I'll get a call telling me what I'm doing wrong, :rolleyes:

So far I'm just writing anything out of the ordinary... Dr appts, results, condition changes, med changes... that are usually written in my narrative, however, the policy says it's for things NOT covered in the narrative?? Except for upcoming appts, I don't think my narrative would be complete without that info, but it seems like the info they want?? Like I said, we're working out the rough spots, I guess. I'd love to hear from an agency that has already has a policy that JCAHO likes!
