Gordons functional health pattern

In my nursing class we have to you this model and turn it in for a project and as assignemts. All I am looking for is a completed one so I can see how it is suposed to look when it is complete. I have my data but I'm not sure how it should be done on the forms. I have tried multiple searches on google but can't find any completed forms. Please help it would be appreciated.

We have to use Gordons Functional Health Patterns when formulating a nursing diagnoses.

There is a page in Kozier and Erbs Fundamentals of Nursing that

has the cues clustered under the different categories.

Like for instance someone who reported " NO appetite" since having "cold" Has not eaten today, last fluids at noon today. Nauseated x 2 days would be under Nutritional/metabolic functional health pattern(includes hydration), this would infer (allow you to make a tentative diagnosis of ) either inbalanced nutrition- Less than body requirements or Deficient Fluid Volume.

Basically you would cluster all your assessment data under the different functional health patterns, and then pick your diagnoses based on the cues.
