Finding a job after disciplinary action

I am an RN that was found guilty of Failure to maintain accurate patient record, failure to follow physician's order, and practice beyond the scope of practice. There was no actual harm to the patient as patient did not receive medication. License was revoked for few years and just very recently re-instated with a letter of concern stating there is low risk of any harm to public. I am very remorseful, realize what I did, and would never place anyone including myself in this position again. My question is I have decided that nursing is what I really want to do and have been looking for a position. What do I tell potential employers? Some ask if there is action against me and some do not. The disciplinary action occurred in a different state than I am currently licensed in.
