FDAR charting

hi! I was wondering about writing an FDAR charting, it's my first time to write one and I've only written a NCP before. When writing the Data for the FDAR, will I only include the ones relevant to the Focus? 

We were given a case where it includes other data like the patient's history in smoking and her family's history. Do I need to include these in writing FDAR?
