I' am a student enrolled in MCPHS's Family Nurse Practitioner program seeking preceptors for multiple clinical rotations for 2016 and spring of 2017. I graduated in 2008 from University of Southern Maine with my Bachelors of Science, Nursing. Since graduating, I've been employed as a charge nurse at a skilled nursing facility, a staff nurse on an adolescent inpatient psychiatric unit, and a case manager with the VNA. Currently I'm employed with a hospice agency as a weekend Baylor nurse. I highly value interdisciplinary collaboration for care planning and take pride in providing patient-centered, evidenced-based care.

My clinical hour requirements and placements needed are as follows:

Spring 2016:

· 90 hours (7/week)

· Class: Advanced Health Assessment.

· Setting: primary care office, outpatient preadmission testing, and urgent care.

Summer2016:

· 180 hours (12-16/week)

· Class: Family Practice (Adult)

· Setting: primary care office, family practice office dealing with patients over the age of 21 years old, internal medicine

Fall 2016

· 180 hours (12-16/week)

· Class: Family Practice (OB/PEDI)

· Setting: primary care office dealing with patient's ages 0-21. Pediatric offices

Spring 2017

· 180 hours (12-16/week)

· Class: Family Practice (GERI)

· Setting: primary care or internal medicine offices dealing with patients over the age of 65, skilled nursing facilities or assisted living facilities that provide primary care to its residents.
