falsely accused

Hello,

I was wondering if anyone can tell me how to handle being falsely accused of abandonment of assignment. I just recently found out that a previous employer added to my file an abandonment/no rehire in my personal file. I did not do this and I am actually shocked to find this out 5 years later. I am currently working agency and was rejected for per diem work for this hospital organization. I was wondering if I could simply send a request to HR to ask it be removed or do I need to spend the money on attorney. I do not have Liability Insurance but I have never had any type of disciplinary action against me nor my license. This is really upsetting to find that a director would actually sabotage another nurse for a personal satisfaction issue. I am also curious as to how much this would cost. It is defamation of character in my eyes and is quite bothersome to me.
