failing test because of nerves or something else?

I am hoping that someone may be able to help find out why I am having such a hard time with CPEs and tests. I returned to school after being out 10 years. I never had a problem when I studied and prepared. Now, for some reason, and it's not for lack of preparation, I take test like a someone who just came in off the street and attempted to get the right answers without knowing anything. I get a sense that I blank out, or that it's not really happening. I am aware that I am not focusing, it seems I can't overcome this. It has cost me dearly because I fail a test after my second try. So, I was abruptly informed that I was dismissed. My grades are good, attendance is excellent. But I feel as though someone put a shredder inside of me and turned it on full blast. Please if anyone can help, please reply.
