Hi, all. I'm a kinda new tx nurse.
My facility texted to all nurses to not put date and initials when doing dressing changes because some consulting services advised them to do so..
I'm very uncomfortable with that.. I would like to continue label dressings because of patient safety and my own safety reasons.
I would appreciate any advice how to handle this .. I can't imagine any rationale for not labeling dressings..
Please, help

The rationale is that then there is no documented evidence that subsequent dressing changes were not performed.

I think it’s fair to assume that if dating a dressing is to record when the last change was done, then intentionally discarding that process is to obscure that information. You have to conduct your practice how you see fit when it comes to safety and positive outcomes. Relaxing on best practices is never about the benefit of the patient. Remember that.
