Document submitted with Declaration of Nursing Education for lvn exam application

Hi there! i am new here and want to ask about this requirement to be pass with the Declaration of Nursing Education form (the yellow one). it says that i have to pass an official copy of my nursing license, certificate pr registration. what exactly these documents are. i graduated in the Philippines. can anyone here enlighten me with this? thank you
