Disoriented Patient suffering of CNS infection

I need some help in formulating two care plans. My patient, 21 years old, has a chief complaint of fever and disorientation and was diagnosed with CNS infection. He's been in the hospital for a month now and is now verbally responsive, though it is still quite difficult to make him respond and is still in need of assistance in daily activities and has recurrence of a slight fever.

So I've come up with:

Risk for Aspiration related to decreased level of consciousness

and Disturbed thought process related to decreased level of consciousness due to disease process as evidenced by...

Am I formulating them correctly? This is actually my third care plan but for some reason I'm feeling a bit lost. Especially because I don't have a NANDA book as reference for interventions. Any help would be greatly appreciated. :crying2:
