As a Case Manager , I discharged a patent to his HD Center. DaVita sent me an acceptance and the HD Center sent me an Acceptance. I thought all was well and good. On the day the patient came for HD ( 2 days after d/c), I heard from the Center that they were sending pt back to hospital after HD as they did not have the antibiotic he neeeded to be given after HD. In my HD packet , I inclouded the script for the antibiotics and the Center asked me to send cultures which I did, now it seems I am taking the brunt of the fault for pt being sent back. I do not understand if DaVita reviewed packet and accepted pt, then sent it to HD Center and they accepted pt back, why may I be written up for this? This just occurred Friday and was just told by my ADN that 'this is bad." I'm sure directed at my role. Has anyone had similar experience?

Update- left that job March 2018. That particular ADN was new to me after working under a wonderful one for 6 years. She was into criticism and intimidation, I finally realized. I got another job using my original ADN as a reference. Right now I have ANOTHER better job! So she can continue her reign of incompetence. Just not near me!
