Does anyone have any basic information on how to do that? Basically, what's the chain of command. I have ideas... Who do I tell?

Your question is not clear. What does chain of command have to do with developing an inservice?
My routine - get SPECIFIC about the topic (too much info and you lose the audience), check out regulatory agencies to see if they have a say in it (JC for instance, or OSHA, CMS---), read you own facility's policy pertaining to topic (after the inservice this is all the nurses have for reference), determine WHO is your target audience (all staff, just ICU?), determine where and when (remember, night people will resent having to come in during the day), then determine how you will evaluate the effectiveness of the inservice (fewer incident reports, improved charting, fewer complaints---). Is that anything like what you wanted with your original question?
My best guess about what you're asking is that if you want to develop an inservice, you aren't sure how to get it approved - is that right? As in, do you need to ask someone in advance, develop it, get approval? If that's what you mean, I don't have to go through that process. I work for a large national company that has an extensive library of presentations developed by the corporate office. However, we also have a lot of latitude at the facility level, and within my facility, I'm given a lot of autonomy with the whole process. So, I'm usually developing things according to the learning needs of the facility at that time. Classicdame's chronolgy is a great reference for the process, and my additional focus is specifying what can be practically applied. For example, when I do a falls prevention inservice, I do talk about etiology and risk factors, but my area of concentration is, "What are the things that you actually can do to reduce falls? What does falls prevention really mean in my job? What are steps I can take to improve this at work?"
