Hi, we are 4th semester nursing students taking a research course. If you are a RN, APRN, or Nurse faculty we would GREATLY appreciate if you would take a few minutes to answer this survey. Dear Potential Survey Participant:

I am a nursing student in the Department/ Division/College of Medicine at the University

of Tennessee at Chattanooga. I am conducting a research to study the factors influencing

autonomy in nursing practice.

I am requesting your participation, which will involve a 37 questions survey, which will

take approximately 15 minutes or less of your time. Your participation in this study is

voluntary. If you choose not to participate or to withdraw from the study at any time,

there will be no effects to your job performance evaluation. If at any time you discontinue

the survey, your results will be discarded. The results of the research study may be

published, but your name will not be used.

There are no anticipated risks, however there might be boredom or emotional distress, so

we included UTC's Counseling and Personal Development Center's phone number

423-425-4438. There will be no direct benefit to participants, however they may benefit

in the future when the information is added to the body of knowledge.

This research has been approved the University of Tennessee at Chattanooga's

Institutional Review Board. If you have any questions about your rights as a subject/
