Critical care case management

I'm a nurse of 18 years mostly in ICU but I switched from bedside to case management about 6 months ago. I feel awkward as a CM in the ICU specifically because these patients aren't exactly ready for discharge LOL. I'm always trying to really figure out how to be of most help to my patients in the critical care setting. Any advice from ICU case managers here?

Not a case manager here. One way our CMs help in the ICU is when a patient who is in the ICU in our tertiary referral center needs to be transferred back to the referring hospital still requiring ICU level of care (when the same service can be offered there and the higher level of care which we offer is no longer required). This is one way to open up limited ICU beds for patients who really need our services. FYI, we do not have a designated Critical Care Medicine CM.
