CONCEPT MAP HELP!!!

I am in need of help!! I am currently working on a concept map on a 91 yr old female, who is in an nursing home. Her reason for being admitted to the facility was due to CVA. She also has a history of anemia, hyperlipidemia, chronic renal failure, malnutrition, dysphagia, end stage dementia, and a hip fracture. WHat I concluded in her head to toe assessment the main problems related to CVA are disorientation, dysphagia, and immobility, I was doing my concept map on CVA I am having a few problems with nanda's the one's i came up with so far are:impaired physical mobility, impaired urinary elimination, impaired swallowing, impaired verbal communication, Chronic confusion, self care deficit,risk for impaired skin integrity. I am just look for some feedback on if I'm going in the right direction and any tips that can help me. Thank you!!
