Communication among nurses and providers

Hi everyone! I'm looking for some insights from fellow nurses on how communication between nurses and providers can impact patient care as well as the workplace. I know every hospital is different but have there been any interventions or fixes to improve interdisciplinary communication that you all have noticed within your hospitals?
