Citrate Protocol for CRRT?

Just wondering if anyone has a citrate protocol or standing orders they use for CRRT that they would be willing to share. This is a new anticoagulation technique for our community. Thanks.

We are working on bringing it to our facility as well. Sorry, no order set yet but I also would like to see some info. I just had a pt clot off 4 cvvh filters last night.

In my experience, citrate does not help much. They still clot off frequently. We use it on our HIT or suspected HIT patients. Or someone with an obvious contraindication to anticoagulation.

The hospital I'm out recently did away with citrate altogether. Didn't have much luck with helping prevent clotted filters & it caused to many issues with labs (specifically calcium) and we were doing ionized calciums every couple of hours & replacing. Now we just flush the filter with saline every 1-2 hours if we are not using heparin.
