I'm writing a careplan for a 23 month old child with MRSA. My diagnosis goes as follows; Impaired skin integrity r/t damaged skin layers secondary to presence of pathogens (MRSA) am/b lesions on right labia, upper right posterior thigh, lower abdomen, with purulent exudate, and positive MRSA culture.

I have to write short term and long term goals. I normally would do education for my short term goal, but she's too young to understand that, so I'm having trouble with that. Can I educate the parent for the short term goal? I'm lost as to how to set my goals for a child this young. Please help! Thank you!

You can absolutely direct your teaching toward the parent. Peds nursing is family nursing, and the parents are integral parts of coordinating care.

However, don't count out the two year old completely. Do they have at least a little understanding of what's going on? Csn they understand not to touch or scratch the wounds? Can they tell mommy and daddy when it hurts or when they go potty so they can be cleaned? You can try to teach the two year old some things as well.

Your medical goals are the same for any age: wound healing, remaining infection free, controlling pain (using an appropriate pain assessment scale), etc.
