Change the shift report sheets

I am new to this form and I read your former post on creating your own sheet for a change of shift but I still have not been able to do it on my computer I'm computer literate but if there's a way could anybody send me a template or at least post one on this site because I'm unable to download it the one that was there thank you so much you guys are awesome
