Case Management in Cardiac Office

Interested in knowing if any one is doing outpatient case management from a cardiac office. Currently working in a cardiac office as a population health nurse starting up a new program and having problems locating other offices doing this same thing. Even though our title is population health nurse we really will function as a case manager with a cardiac focus. I came from an outpatient chronic care management job that had a focus in the primary provider's office where we helped with anything and everything. However, trying to put a focus just on cardiac may be difficult as each disease can effect the other. Is anyone doing cardiac case management in a cardiology office?
