Care Plans

I am trying to find some pt scenarios for doing care plans, with the correct care plan to check my practice. Does anyone know of any websites that have good Pt scenarios? I have checked my med surg books and none of the scenarios have a care plan to check against what I have. I am an online student, and there isn't a lot of support for checking care plans.

1) As a nurse, what is the expected medical diagnosis?

2) Based on the medical diagnosis, write down in order of priority what nursing diagnoses you would expect?

3) What are the goals and expected outcomes for the patient to meet discharge requirements?
