A little about my patient: Patient is total care. My nursing dx for him is Self Care Deficit dressing, grooming, toileting, r/t Hemiplegia, Demenita and CVA secondary to musculosteletal impairment, AMB inability to perform ADL's
If this dx is suffice, I need a broad goal, and outcome and three nursing intervention. Ive been trying to get a handle on this care plan thing all semester. I ask lots of questions and my instructor answers me with a question. It's mid term now and I NEED HELP:crying2:
I appreciate any and all help.

i hated care plans and sucked at them until i bought these nursing diagnosis cards @ barnes and noble or somewhere. they had the relevant assessment info for each diagnosis. they were really helpful. but i still hated care plans.
