care plan... Nanda: Pain, Acute

I have to create a careplan out of a scenerio and i have no idea where to start, the scenerio has no subjective or very much info, here it is:.....

35 y.o. male with uncontrolled migraine headache complains that the pain is causeing nausea. IV analgesics have been administered by the RN hourly as ordered.

Its a mini ICP with SOAPIE form....can someone please help and suggest where i should focus my attention on...the migrain or the nausea??

Since he says the pain is causing the nausea, I would focus on the pain. No pain = no nausea, if what he says is true.

Also, in the absence of any life-threatening problem (ABC's), pain is going to be THE priority nursing diagnosis no matter what.
