Care Coordinator v. Case Management

Hello - I am considering a position as an acute care coordinator at a large medical center.  I would eventually like to travel when my kids are grown and it's just myself and my husband.  I only see travel jobs in case management.  Some say the terms are interchangeable while others say they are different.  My question is, would working as an Acute Care Coordinator qualify for a position in Case Management? TIA
