CAP project ideas for work

I work a tele med surg floor and wanting to sign up for a CAP project . We use Cerner as our EMR. I would have to submit my proposal on what I want it to be . Im thinking something like continuity of care from admission to DC (floor to door) I would need ideas and then write out policy for it.  Such as the same nurse stays with the same patient for continuity of care . The nurse will give the PCT report.  The nurse will have such amount of time to review ER charts for proper admission etc.   I would have to have professional sources etc to back up the proposal such as the such and such journal reports decrease in mortality rates did the same nurse is with the patient as you'd catch subtle changes

I've also thought about something like proper admission criteria for the floor such as certain labs or vitals would go to PCU - we can't do continuous bipap, high flow o2, trachs on our floor

 

 please please help me out
