California: DEA and Malpractice

Hi all,

I just got hired for my first NP. I am thinking to open an S-Corp for the tax purpose. When I apply for DEA should I put it under my name or the S-Corp (SSN or TAX ID?). Also, the same for the NSO Malpractice, do I open it under personal name or the S-Corp?
