CA NCLEX reciprocity to NY

I have been reading this forum and find it very helpful.

I am a licensed registered nurse here in the philippines and has passed CA NCLEX exam. However, since I have no US SSN, I still don't have my license. I'm planning to apply for NY BON so I can get a license and avoid the 3 year purging of file. Upon reviewing the NY BON application packet, I have some questions that I hope other members can answer.

1. In Nurse Form 1 section 18, do I tick Yes for the question "Do you now hold, or have you ever held, a license or certificate to practice any profession* in any jurisdiction? ". I have passed the CA NCLEX exam but does not have a license due to the SSN requirement.

2. Referring to my first question, does NY BON needs the license verification of my license here in the Philippines? Nurse form 3 contains only the NCLEX and SBTP exam scores and it is not applicable here in the Philippines.

3. It is also required that my signature in Nurse Form 1 be notarized. Is it okay if the notary public that will sign is here in the Philippines?
