Hi, I was just wondering if anyone knew of any books that had care plans set up by diagnosis instead of symptom??

Clinical Applications of Nursing Diagnosis: Adult, Child, Women's, Psychiatric, Gerontic, and Home Health Considerations by Helen C. Cox 4th edition ISBN 0-8036-0913-2
We are using this one along with Mosby's Medical Dictionary for our care plans and it seems to be easy to find what I need. Hope this helps!

Thanks, Is this book hospice based or general care plans?
I think it's general care plans? I'm not too familiar with the differences so I can't say for sure, but since I'm not specializing in hospice care, it's probably for general care plans. I do know it has a section on Home Health Care.
Thanks!
