Blood when straight cath

Hello, I'm a new grad nurse ! I had a patient today that is retaining a lot of urine, had the catheter removed and was straight cathed four times for the day. The previous time he was tensing up and then when he finally relaxed it advanced. This next time as I was advancing I met resistance, I instructed him to relax, but I still couldn't advance it. It was a coude catheter that I made sure was facing up when inserting, so I tried to reposition it if it got switched. We ended up removing that one and doing another straight carb and when we removed there was blood. We wiped it away and straight this time we're able to get into the bladder, but I immediately felt so terrible. I feel horrible for causing my patient to bleed and feel like I'm a bad nurse. Luckily after removal there was no further bleeding and my preceptor ensured me it wasn't enough to be a concern, but I feel so horrible for this.
