Hi!

I have been an OR Nurse for 6 years and I was recently approached by a Surgeon to become their First Assist. They only need me on a per diem basis. I think it is in my best interest to be a contract RNFA. Does anyone know of any good resources in regards to creating your own employment contract and how you would go about billing the Surgeon? I live in NJ!
