I was wondering if anyone had any examples they could share with me. I just started nursing school and I will be given something on the test that says like:
"maria came in with a temperature of 103, she had an infection in her throat. she said on a scale of 1 to 10 the pain in her foot was a 10."
and they want me to write: hyperthermia DT an infected throat AMB a temperature reading of 103.
I cannot, for the life of me, find examples like this on the internet. I find lots of nursing care plan examples but when I've obtained the information, I want to work at them at this beginning stage to make sure I'm 100% proficient.

I'm not sure what type of book you're looking at. Are you looking at a nursing text or a care plan book?

Often thhe best resource for care plans is your fundamental or med surg books as they often offer up sample care plans based on topics covered throughout your semester

Try RNcentral.com, they have a few NANDA diagnosis with associated care plans. Daytonite is really the one that should be answering here as she is the care plan queen but until she does I can offer you this:
1) first part is what does the patient have....in this case, a fever.
2) d/t is why does he have it?
3) AEB or AMB how do you know he has it?
So if you can ask yourself all of these questions with any given scenario you should be able to write an effective diagnosis. Lets look at one.
You just admitted a patient to the ER with nausea and vomiting. The patient states they have not been able to eat for 2 days because they "can't keep anything down". They complain of being tired and so weak they can't get out of bed.
So ask yourself the first question.....what does he have?
1) Nausea and vomiting, yes but we cant move on to #2 cause we dont know why he has it yet. So lets think about what would happen if you had N/V. You obviously are not getting enough nutrition if you are puking constantly. So we have:
a) Impaired nutrition
2) So if we have impaired nutrition....why do we have impaired nutrition?
b) Impaired nutrition d/t nausea and vomiting
3) How do we know he has impaired nutrition? because he is fatigued and weak. (this is your evidence)
c) AEB or AMB fatigue and weakness
So your final would be.....Impaired nutrition d/t nausea and vomiting AEB fatigue and weakness
There are a bizillion other diagnosis that can go along with this example as well. Such as Risk for fluid and electrolyte imbalance etc. Once you have all of the diagnosis you will want to prioritize them based on Maslows (well, thats what we have to do) but I will stop right there.
