Another combining assessment

Had a resident return to the sNF, received Pt/OT then was discharged 2 days later back to hospital. I tried to do a 5 day, SOT and discharge RA together. I am getting an red error and default rate for RUG. Can I combine all of these? Admit 11/17, DC to hospital 11/18, received therapy starting 11/18 through 11/19. 0 (not an OBRA), 5 day PPS, Start of therapy and DCRA with aRD 11/19. I want to do this as a short stay assessment but when I do the assessment section Z has a "no" under short stay and I can't change to "yes". Is this a software problem or am I coding incorrectly. I'm a little confused. Was the resident readmitted to facility on 11/17, then get therapy for two days (11/17 and 11/18 or 11/18 and 11/19), then d/c back to hospital on 11/19? If so, couldn't you just do a readmit combined with a discharge the day they returned back to the hosp? Or short (haha) five day if they were just picked back up by mcr, combined with discharge? providing I guess that they received therapy the day of discharge. anyone else? i need a 3.0 manual for home use, but they are so darn big.

Resident was readmitted 11/16 on MA. Starting to receive therapies on 11/17 and also rec'd 11/18 and was discharged that evening back to the hospital. I did an entry for 11/16. Can't I do a 5 day, start of therapy and discharge along with a short stay (to get my nursing and rehab rug) with an ARD of 11/18? Or do I need to separate them somehow?

From what you are describing, it sounds correct. 5day/SOT/discharge with short stay. Did you make sure to dash the end date of the therapy on sect. O?

Maybe it is your software.

You don't need to do the "Start of therapy" portion if they started back on therapy by the second day of admit, so you can do the 5 day as a discharge. The software should allow that...

Try coding it as a 5day /SOT and EOT/DC then enter the start of therapy date and end of therapy date in section O. It will be a short stay assessment.

What is your software? We use pointclickcare in my facility and it sucks.

WE use Accu-care. It looks like I got several different opinions on what to do. SOT or no SOT. I'll look at this more tomorrow. thanks for the input.

should only be a 5 day assess combined with a D/C return anticipated.........no need for SOT or EOT and not a short stay.

Happy Monday, at least it's a short week..........which only means work later to be able to take thursday, friday off......still have to take friday as a vacation day, even though i'll probably be there untill 8pm tonight and tuesday...
