Hi all,
a bit of a bizarre question - I have been asked to review and revise our current acute adult admissions paperwork and I'm researching other hospitals paperwork and wondered if you guys could help me out by detailing what forms you complete when a person gets admitted onto your acute adult admissions psych ward. I'm based in the UK so are goverened by the Care Programme Approach so not a question for overseas folks I'm afraid (although please feel free to reply anyway as being a point of interest!) CPA documents and risk assessments seem to differ widely and this is what I'm mostly interested in.
Also - in what format do you write your care plans?
Do you see the point in HoNos? (despite it being an intergral part of CPA)
Do you use contracts on admission? and if so what sort of points do you cover in them?
Also I dont know how practical or feasible this would be but would it be possible for people to scan their documents and e-mail them to me to print out to have a look at?
What do you think of admission paperwork in its entirety? Where I work - we currently have over 20 different forms to fill in - all of various pages long which I think is absurd and am wondering how effectively to cut it down without compromising the quality of the information we document.
Any help would be greatly appreciated.

I no longer have forms to fill out. My admission database is on the computer and it's called EPIC. I hope that you find what you are looking for.
