Acuity tool for pediatric home health

A colleague of mine who visits medically fragile foster children is trying to develop an acuity tool. We are looking for a sample acuity tool that we can use as a template, preferably free or low cost. Does anyone know of anything? We've already searched the archives and found one very old thread (1998) with not very much information.

Alternatively, if any of you use an acuity tool in your home health practice, could you please describe how it works?

Just thought I'd add that we'd be interesting in hearing about any home health acuity tool, it doesn't have to be specifically for pediatric patients.
