Academic Forfeiture, Jeff State Or Anyone?!

HI ALL,

THIS IS FOR ANYONE WHO HAS/HAD A G.P.A. BELOW A 2.5 & COMPLETED A ACADEMIC FOREITURE FORM FOR NURSING SCHOOL. IT IS LISTED ON THE NURSING APPLICATION & THIS FORM IS AVAILABLE IN EITHER ADVISING OFFICE. I HAVE (2) QUESTIONS:

1) WHAT ARE THE DETAILS OF THIS FORM ( WHAT IS NEEDED TO GET APPROVAL?

2) WILL I HAVE TO GET THIS FORM DIRECTLY FROM THE ADVISIOR, OR IS THIS A FORM THAT CAN BE PICKED UP OUTSIDE/NEAR THE OFFICE? FOR EXAMPLE, COULD I PICK UP THIS FORM AFTER 5 P.M.?

FOR THOSE WHO KNOW WHAT I'M TALKING ABOUT OR HAVE COMPLETED THIS FORM, FEEDBACK ON YOUR EXPERIENCE WILL BE GREATLY APPRECIATELY.

My grades were low b/c of the grades I had a UAB.. My second application I choose to forfeit my grades from UAB.. But you have to forfeit all grades from that school. If there are classes you need, then you have to retake them. The advisor will give you the form, i'm not sure about picking it up after 5pm, maybe you can email her to see if she can leave it with the front desk. The form is maybe one of two sheets, you are basically just signing that you are willing on forfeiting those grades. There's no guarantee they will approve it, it's just saying you are willing on giving them up. When the advisor and the dept head, etc sort the next applications they decided then if your forfeit is approved. Hope that helps

THANKS SO MUCH, THAT'S EXACTLY THE INFORMATION I NEEDED. WOW, THAT HURTS I WORKED TOO HARD FOR GRADES & I ALSO GRADUATED FROM UAB & ALREADY HAVE ALL MY PRE-REQS. TAKING A BIG STEP BACK IS SOMETHING TO THINK ABOUT. HOWEVER, THAT'S AGAIN, & GOOD LUCK TO YOU!
