was this for the verification of license?

you should have 2 forms;

1 for philippines

and the other one the origin of your NCLEx state, of where you originally applied your NCLEX eg california

I need your help badly! I recently passed the NCLEX-RN in Arknasas but haven't issued a permanent license due to SSN and so I want to have my passed result be transferred to TEXAS. Here are my questions:

1. Will I send my application first and later send the VOL forms coming from the licensing agencies?

2. Can I use the FBI fingerprint card issued by the US embassy in Manila even if the "ORI" part is blank?

3. My biggest dillema is this, I e-mailed the AR BON to know the process of obtaining a letter and also the filling up of the VOL form. However, they replied with this: "The only “letter” that we send is that which you have already received." I received 2 pages from them: 1. letter telling me that I did pass and that they're requiring me to have a SSN. 2. NCLEX_RN candidate form.

I am really confuse on what to do. Esp. on no. 3.

********************************

1. send the endorsement application together with the fingerprint cards, payment and the photocopy of your letter from the original state of your nclex stating that you passed...

with the verification of license, that is done thru the involved offices in your country... if you are not from the US you should have 2 verification of license, so before you try to send out your application of endorsement to bon texas, make sure you try verify your license ahead of time, like 2 weeks to a month in advance to the designated states...

2. for the fingerprint card, you should try to ask from the bon texas...

3. fill out first the verification forms from texas ... there is also necessary fees before you can ask from them to verify your license from their state. and also include inside it a photocopy of your letter from their bon of you passing nclex

for the verification you will have 1 copy each for california and the other one for PRC in the philippines.

for california verification questions, answer the following:

a) california, USA

b) date of your exam

c) you will leave this blank since a license number cannot be issued.

how about the copy for PRC (Philippines)??? will I leave ds items a, b & c blank??tnx

**************************

for the prc, you will answer still a, b and c..dont leave it blank...

a) philippines

b) check the date on your prc card; it should state there the date of registration

c) put their your license number

this refers to both actually

that is why you are supposed to give out 2 forms for the verification of license

and on the endorsement of license you will write california USA and philippines...

I am so so confused on how to fill the form because I am an International graduate Nurse. The form requires me to fill my original licence number, origial state of licensure and date issued and california is not my origin of state Licensure and Ca. wants me to fill in my Ca. Licence number. Can I fill in Ca. Licence Number on the space provided for original Licence Number?

there is really no need to be confused. from what you are saying is since you are a foreign graduate you are to verify from the 2 original licensure agency that you have. first would be the original country where you graduated from and the 2nd verification is to the BON california... that's it...

yes you will still put on the ca license number on the space provided for your original license number.. and on the form where you are required to put both of the original license state just put the 2....

Hi everyone.

My question is, if I haven't found an employer yet in Texas can I still apply and have my license endorsed from CA to TX?

Thank you.

Why does it take sooo long to Verify a License from California to Texas?

i don't think, it takes long to process the endorsement. it takes up to a month to get the temporary license and of course, you need to take the jurisprudence exam as well when you get your temporary license...
