I am trying to find a breastfeeding care plan for a baby with Down's Syndrome. I am a Certified Lactation Consultant, but we have a small population.

I am not sure why you are having difficulty finding resources. Google "breastfeeding Down Syndrome" and lots of resources appear. In addition, you should try the LLL site, Dr. Jack Newman's site, and ILCA.

As an aside, but an important one since the distinctions must be made clear, a CLC is a lactation counselor. An IBCLC is a lactation consultant. There is a HUGE difference in training and experience requirements between the two.
