I am writing a paper due tomorrow on fluid and electrolyte imbalances from a client we selected in clinical. We have to come up with two nursing diagnoses, one for fluid imbalance and one for electrolyte imbalance.

However, we cannot use fluid excess/deficit/ hypo or hyper. Same with electrolytes (no excess/deficit/hypo/hyper).

The topic is CHF, and I was able to come up with one diagnosis (activity intolerance related to decreased cardiac output) for fluid imbalance. Just need some brainstorming ideas for a second diagnosis.

How about something like "ineffective tissue perfusion" or "decreased cardiac output"? Maybe "acute pain" or "impaired gas exchange"? These are just things related to heart failure. Hope it helps!

I was thinking about the things CHF does and went from there. Don't have my NDX book in front of me, but in addition to what PP mentioned, off the top of my head they may be short of breath or have peripheral edema (think about skin integrity, perfusion, ability to ambulate, etc). Good luck!
