Need help with a Nursing Diagnosis for a comatose DNR patient

I have (well, had) a patient that I need to complete a nursing care plan for after the fact of caring for her. She is pretty much comatose and unresponsive to painful stimuli. She's very old, with acute renal failure among other complications. Her family updated her status to DNR, so she will not be dialyzed and g-tube feedings have been d/c'd. I know that I need to provide comfort measures, turn her every two hours and provide for any pain relieving measures. What I can't figure out is what nursing diagnosis to I assign in this case. All end-of-life NDx's seem to address dying patients who are conscious and dealing with psychosocial issues, but that is not the case here. Can someone help me nail this down?

There are lots of possibilities.

You could try "Risk for impaired skin integrity" and expand it from there.
