Needs Assessment for Acute Care Hospital

I work as an Organizational Education Coordinator for a local acute care hospital and need to develop a "Needs Assessment" survey tool for the entire nursing staff. Is there anyone out there who has this tool and is wiling to share? I would greatly appreciate it so it would save me time and wouldn't have to re-invent the wheel.

I am in graduate school in my second education practicum. I need to develope a needs assessment to give to a 30 bed medical surgical unit with RN's, LPN's and CNA's. Any idea of where I could locate a template to design such a tool. Also if you know any refernce books or articles that would be helpful I would appriciate the names of these also.
