credentialing

OK i have a credentialing form to fill out. I did not have that much interaction with the program director but i did make some waves my last semester, my committee chair person dragged her feet I could never get her to be timely on returning my thesis back for corrections I got fed up and went to the dept chair and barked up her desk it got to the point that i was ready to quit I wound up graduating nine months late because the committee chair for my thesis dragged her and now the credentialing form is asking to list my program director I am worried that the program directer will rate me poorly on professionalism because I was very insistent when things were not getting done in a timely manner. I never said anything derogatory just comments to the effect that this was being handled poor I had documents misplaced for my thesis or lost and after the 5th time let my chair know thoughts any one
