CMS Forms 672 and 802

Who completes the CMS Forms 672 (Resident Census and Conditions) and 802 (Roster Sample Matrix) in your facilities? This was recently added to our duties in our MDS office, and we are to update these daily. Can anyone give my any hints for this?

In every facility I've worked in, the DON/ADON did this & I'm thinking this was something they focused on when the facility was in their window for annual survey. Contact your computer dept. for assistance.

In my facility it is the RNAC that keeps it up to date with input from SS, Restorative, and Dietary. Our MDS software can print one out for us, but it's not as accurate. We keep an excel spreadsheet on a shared drive that everyone can get to. It is the actual CMS forms. Right now they are responsible to make sure it is updated weekly and I pull it for a QA report I present monthly. As we get closer to the survey window, it would be updated daily. Hope this helps.

That is a good idea, updating it weekly and then daily during the window period - thank you for the suggestion. What I have done, and it may be overkill but I don't really know another way to keep it straight, was put the questions for the 672 across the top of a spreadsheet, then the resident's names down the lefthand side. That way I know who I am making changes on. By putting a 1 in the column instead of a checkmark, the program can total each column and it is nothing to transfer that info over to the actual 672 form. However, updating the info each week/day is one added task I didn't need!

In my facility MDS coordinator does weekly except when in survey window done daily. It is a pain but when survey walks in and I have it ready I feel much better than when I have nothing prepared and have to try to figure it all out and get it back to surveyers in 15 min :)

If you have a stand up meeting where you go over 24 hour reports, new orders, falls, etc....that is a perfect time to bring it with you and update it. We have stand up daily where we discuss all of the above plus anyone newly started on ATB or psychotropic meds. Every monday we run reports from our software which tell us who has foleys, tf, resp tx, o2, etc....at that time we update our 672/802 and also check for foley justification/POC, restraint/enabler assessment/POC...etc. Makes for a long meeting on Mondays but we have had 2 perfect surveys in a row so thinking it must be working :)

I am a nursing home administrator and was DON for many years. This is something that is completed by the DON/ADON along w the help of the MDS nurses. We normally start these forms 3 months prior to our last survey.
