Flow sheets/check lists for floor staff?

How do you all keep your floor staff organized with all the bits and pieces that need accomplished? We are having trouble making sure that all the appropriate assessments, skin sweeps, and behavior grids are getting documented as scheduled. We have different sheets at the nurses stations for the nurses to look at and initial when complete, but often the sheets are left blank and I have to hunt through our computer system to find out if things were completed. Any suggestions? Also, how to improve accountability?

Set up a standing file folder box with a folder for each nurse OR set aside shelf space and stack a binder for each nurse on it, clearly labelled. For each shift's nurse, place any forms needing filled out or checklists needing filled for that shift, especially ones that seem to get neglected. You can also place any daily memos or notes about upcoming meetings or in-services in there. Require that these papers be turned in to a door box ( you know, one of those things that can be mounted onto a door to hold papers and such- I don't know what they're called.). That way, even if you're not in your office, they can turn them in to you and there's your accountability. Back when I was a CNA, we had to turn in a sheet detailing that every resident had been charted on to the door box of the nurse manager. If you didn't get your paper turned in, look out!
