I am curious, do any of your agencies have a nurse that is dedicated to only doing the OASIS either at start of care, discharge, and/or recertification? How does this look? WHat is the had-off process for the case manager assumign care? We currently have our case managers dot heir own, but we are considering this shift thinking it might improve our outcomes.

Hi, I've been a SOC nurse for 5 years. I only open 3 cases per day, hourly employee. I send an email report to the supervisor who then forwards the report to the case manager. I make sure to confirm the orders with the doc, obtain the follow up appointment with pcp. It's actually getting to be too much for me because of all of the paperwork I have to do and I'm always behind because of the volume of work needed for each start of care. I'm planning to switch back to case managing. I was initially offered this position because I had good hhrg scores with the Medicare patients, and a lot of the nurses don't have good scores, but they are constantly training to improve the numbers.
