Advice on resume for HHC Transfer.

I've been a nurse for almost 6 years now, 5 years in CVPCU with post op open hearts and thoracotomies being our main population as well as CEAs and vascular surgeries. We also got a little bit of everything as overflow though...you know how that goes. I then transferred to our level II trauma center's ER (the same hospital) as a per diem nurse. After almost a year leadership and I have come to the agreement that with the amount of time I'm able to dedicate to work right now (1 day a week, 12 hour shifts), it's just not a good fit to be able to garner enough exposure to our critical patients to become proficient and comfortable. So...I have 1 month to obtain a transfer within our organization and they will help me anyway they can but there is only so much they can do. I have no desire to go back to the PCU, not after having a taste of something different and loving it so much! My plan was always to move to home care, I just didn't think I would be doing it for another 5-7 years after my daughter and future baby start school. Well now I'm putting in a transfer to our home health department for a per diem position. I've done a lot of research on home health in general and am prepared to put in the time for full time orientation and then go down to per diem. I've spoken to one of their case managers that's a friend of a friend and I'm ready to do this! The hurdle of course, is getting hired within a month by their hiring manager who is really busy right now because they are fast growing and are having growing pains from what I understand. Anyway... Help me with my resume please! I haven't even started my cover letter but will whip one up some time today so I can fill out the transfer paperwork and get this ball rolling. Any advise on what to include there? If I don't get this transfer on time I will have to apply as a new hire, which stinks and is a lot more paperwork. Changes I just made after reading it for the 5th time!

Changed "or" to "and" between PEG and NG tubes.

Changed tense of last point from "Interpretation of telemetry" to "Interpreted telemetery."

Should I change "Collects specimens, including blood from venipuncture and fingerstick as well as urine from in and out catheterizations and viral and bacterial swabs from nares, throat and wounds." to "Collects specimens, including blood from venipuncture and fingerstick as well as urine via catheterizations and viral and bacterial swabs from nares, throat and wounds."?

This exposure has fortified my assessment skills as well as sharpened the skill set I already possessed. While I love working in the department, I am unable to put in the kind of hours long term that would satisfy the department’s needs and give me enough critical care and trauma exposure to really feel at home there.
